Q: I want to come out for a varsity sport. What do I need to do to be cleared for participation?
A: NCAA requires that all student-athletes that have not previously participated in a Varsity sport must complete the following. This includes transfers.
- Pre-Participation Physical Exam Form. Prior to participation in any practice, competition or out of season activities, student-athletes who are beginning their initial season of eligibility shall be required to undergo a medical examination administered or supervised by a physician (e.g. family physician, team physician). The examination must be administered within six months prior to initial participation. This form needs to be signed by a physician (M.D., D.O.)
- UW-Stout Medical History Form signed by physician
- Insurance Form with current insurance information and emergency contact numbers. Insurance must cover athletic injuries and have coverage up at least $75,000. This includes copies of the front and back of the card.
- Acknowledgment of insurance requirements form signed by student-athlete and their parents.

Q: I am a current student-athlete at Stout. Do I need a new physical each year?
A: Student-athletes that participate in football must have a physical prior to every season. A physical for all other sports is only required prior to initial participation. However, all student-athletes must review and sign their medical history that is on file in the athletic training room prior to every season.
Q: I have recently had surgery, a visit with a physician, physical therapist, chiropractor or other allied health professional back home. What paperwork do I need to provide to be cleared for participation in a varsity or club sport?
A: One of the issues that sometimes arises during the year is that of appointments that have occurred away from campus. If this is your case you need to supply information so that we are fully updated to your situation. Items such as surgical reports (ie. what the surgeon did), physical therapy guidelines, and follow-up notes from the physician noting progress or return to sport. Please understand that simply providing a note that states you may return to sport in place of the above paperwork is not sufficient. The team physician has the ultimate say as to your participation in a varsity sport.
Q: What if I get ill, such as with the flu, or have a non-athletic injury while competing at UW-Stout? Who do I see for care?
A: Always contact the athletic training staff first with any injury or illness. We will refer you to either our campus student health center or proper medical physician for treatment and care of your illness.
Q: What are the hours of operation of the Athletic Training Room?
A: Typically, a certified athletic trainer is present from 1:00 to 7:00 pm Monday thru Friday. The athletic training room is open during the morning hours although the athletic training staff may not be available due to teaching responsibilities. Weekend hours may vary to accommodate sport practices. We will do our very best to post hours if they change.
Q: What if I am an athlete and my parents don’t carry health insurance on me?
A: All athletes must have healthcare insurance coverage that covers athletic injuries. Most students get insurance through their parents’ policy. There is no secondary insurance policy provided by UW-Stout. UW-Stout does not pay for student-athlete’s medical bills.
Q: What if I have questions about supplements I am taking?
A: Always bring in any supplements you are taking to the athletic training staff. We can look up certain ingredients or contact our team dietician. You are responsible for making sure that all supplements you ingest do not contain NCAA banned substances.
Q: If I am in a club sport or play intramurals, can I utilize the athletic training room?
A: The athletic training room and supplies are only available to varsity athletics. If you do sustain an injury in a club sport or intramural, the athletic training staff can guide you in the right direction for proper medical treatment.
Q: Are there student jobs available in the athletic training room?
A: There are jobs available for students who qualify for work study. Please contact Susan Lew, head athletic trainer for availability.
